Director of Population Health Management [closed]

The Director of Population Health will be responsible for the implementation of population health initiatives targeting specific disease groups and the coordination of education, services and care that can have a positive impact on the health of defined population groups. Under the general guidance of the VGH CEO, the Director will develop beneficial programming that positively impacts community health, understands the financial implications of improving health for the individual and the organisation, and increases community involvement in vital health and social issues through building partnerships with governmental agencies and community services providers. The Director will develop strategic initiatives and lead implementation to ensure the population health management program operates effectively and is well coordinated among participating care providers.


Duties & Responsibilities will include:

  • Provide leadership, motivation and direction across VGH to ensure that the organisation supports best practice population health practices
  • To set goals and evaluate the impact of clinical interventions and monitor progress, as well as creating a feedback loop that informs all stakeholders regarding plans, activities and results
  • Provide supervision, direction and leadership to staff in the delivery of service
  • Develop, implement and evaluate VGH Population Health Programs, in line with the National Health Systems Strategy for Malta 2014-2020
  • Take the lead role in the development of the Population Health strategy whilst assisting in implementation of intervention change management programs and evaluate program success
  • Demonstrate leadership and management skills to effectively communicate organisational strategic directions and implement action accordingly
  • To abide by and actively support VGH policies to ensure the safety and wellbeing of employees, clients, contractors, patients and visitors
  • Collaborate with all Stakeholders across the organisation in order to promote the pursuit of quality practice and innovation in service delivery
  • Provide guidance on how VGH can develop and implement solutions that advance continuity of care with particular focus on reporting and data analytics to help manage the costs for both primary and secondary health management
  • Actively work with VGH Management and Leadership team across the organisation to facilitate operational management of the service, including integrated service delivery, planning, quality assurance, risk management and occupational health and safety
  • Identify training needs and facilitate staff participation in relevant training programs in conjunction with the HR Department
  • Developing costing and financial plans in conjunction with the CEO, COO and CFO for operation of new and current services
  • Use data analysis to identify and implement opportunities to improve health outcomes, claims experience and lower medical costs
  • To analyse baseline measures of treatment costs and health status, and the tracing of costs and outcomes over time.


Skills and Experience:

  • Minimum 5 years’ experience preferably in health population management, public health or relevant field
  • Leadership and management skills to effectively communicate organisational strategic directions and implement action accordingly
  • Sound experience in the management of a diverse workforce, including staff management and supervision
  • An excellent understanding of population health principles and community development practice
  • Sound experience in planning, implementation and evaluation of population health programs
  • Strong background in analytics, program and project management
  • Experience in managing projects across multiple entities/departments with various stakeholders.



  • A Medical Doctor registered with the Malta Medical Council
  • Specialist in Public Health listed on or eligible for inclusion on the register of specialist doctors kept by the Medical Council of Malta or in possession of a certificate of completion of training (CCT) issued by the Specialist Accreditation Committee of Malta, which shows that the applicant has completed such specialist training as may be prescribed for Public Health.
