Patient Pathway Coordinator [closed]

The position of Patient Pathway Coordinator is required to formulate, analyze, monitor, and evaluate patient pathways.  The Patient Pathway Coordinator also ensures that the patient pathways are implemented effectively.  The Patient Pathway Coordinator has to liaise and consult with the expert leads of the relevant clinical streams across all levels of care in order to provide patients with a holistic seamless service.

 

Main duties and responsibilities include:

  • Provide the Director Integrated Care with strategic planning and monitoring of integrated patient pathways;
  • Advice the Director Integrated Care on the setup of multidisciplinary patient care pathways, involving all interested stakeholders as necessary;
  • Develop and implement pathways that help navigate the patients through the different health services;
  • Plan and coordinate a patient navigator system that provides personalized guidance and multidisciplinary care to patients.

 

The selected candidate should:

  • Possess a Masters qualification in a healthcare field issued by the University of Malta (or any foreign institution duly recognized by the UOM), certified by MQRIC at level 7 or higher in the subject;
  • Have a at least 3 years’ experience in developing clinical processes and clinical pathways;
  • Have a at least 3 years’ experience in managing a healthcare multidisciplinary team;
  • Very good knowledge of integrated care pathways;
  • Possess leadership skills;
  • Be assertive with excellent communication skills both verbal and written;
  • Possess strong analytical and decision-making skills;
  • Be flexible with a positive and can-do attitude.
